What does “bespoke” mean?
First, we must understand the origin of the word “bespoke”. A hundred years ago, clothing was hand sewn by a tailor for each individual client. A client would first pick out the fabric, from which the tailor would create a garment. Later, if another client came to the tailor’s shop, they could not pick the same fabric, because that fabric had already been “spoken for.” The fabric was “bespoke”.
Crafting the perfect fit: the art of plastic surgery
Plastic surgery has many analogies to tailoring. Loose sleeves can be tightened. Excess in the waist? Pants can be trimmed. In tailoring, extra cloth is removed and a seam is placed in an aesthetically acceptable location. In plastic surgery, a brachioplasty removes excess skin from the arms and the “seam” is hidden on the inside of the arm. A tummy tuck removes excess skin between the belly button and a c-section line. A facelift removes excess skin and tightens the face. A Bespoke Facelift consultation is different in that the patient leads the discussion. The patient may discuss their top three concerns such as jowls, extra skin, wrinkling, and lines. Another patient may note their top concerns as being the neck, cheeks, or eyelids. The facelift must be custom designed for each patient: this is bespoke.
We once had a patient who came to our office after visiting several other facelift surgeons. The other plastic surgeons explained how they did their facelift. One surgeon mentioned the deep plane, the other surgeon discussed the SMAS, and another talked about the short scar facelift. But the patient didn’t want their facelifts, she wanted a bespoke facelift. She explained that she was troubled by the position and size of her ear lobes in addition to facial rejuvenation. The other plastic surgeons explained that “ear lobe modification was not a facelift procedure.” This is technically true, but not relevant to the patient. They other plastic surgeons did not understand bespoke.

The bespoke facelift requires creativity and flexibility. Bespoke means tailoring a procedure to the patient’s goals. If an upper or lower eyelid procedure is added, planning must be done to ensure a natural result. Some patients choose to have fat grafting to improve skin tone and warmth as part of their bespoke facelifts. Other patients primary focus of their facelift to be improvement in the neckline.
