Do I need to lose weight before abdominoplasty?

Written by Mark Domanski, MD

What is a healthy weight for abdominoplasty?

This is a frequent question that patients are afraid to ask.  First, what is a healthy weight?  There is a ratio of height and weight called body mass index (BMI) that is useful to understand.  Simply put, a 5-foot tall person is considered overweight at a lower weight than a 6 foot tall person.  To calculate your BMI, type “BMI calculator” into your search engine or check this BMI calculator.

A BMI 25 to 30 is considered to be overweight.  With increasing BMI, surgical risk does increase.  Above 33, many plastic surgeons may hesitate to perform an abdominoplasty.  Patients may still be a candidate for a panniculectomy.

A Practical Approach to Tummy Tuck

BMI is not a hard and fast cutoff.  For example, a 25-year-old patient with a BMI of 35 will recover better than a 65-year-old with the same BMI.   Some patients are truly big boned or have larger muscles.  An active patient with a higher BMI will do better than a sedentary patient.

Now let’s talk about reality.  We need to live in the world that exists, not a perfect world that does not.  Many patients benefit from an abdominoplasty at higher BMI.  However, the results and recovery are better at a lower BMI.  Some patients try medical weight loss, or GLP1 inhibitors like Ozempic to lose weight prior to an abdominoplasty or tummy tuck.

Importantly, weight should be stable for several months prior to undergoing an abdominoplasty or tummy tuck.  This ensures we are not chasing a moving target.

There are options for patients with higher BMIs: for example, we may choose to observe the patient overnight, or use a different fluid management protocol.

Weight and Surgical Safety While Considering Abdominoplasty

In conclusion, weight loss before an abdominoplasty may be beneficial, but it also may not be necessary. We are well acquainted with the issue of weight loss and plastic surgery and can help advise you during a consultation.
