Chin and neck liposuction (also known as submental liposuction) is a common, outpatient procedure performed under local anesthesia in the office. Most patients are able to return to work within a few days. The minimal procedure offers a safe, precise way to sculpt the lower face and achieve a more refined, youthful appearance. It is especially effective for patients with stubborn fat in these areas that doesn’t respond to diet or exercise.
Informed decision-making: The first step is to set up a consultation with Dr. Mark Domanski, a board-certified plastic surgeon. Dr. Domanski will listen to your goals to see if chin and neck liposuction is a reasonable procedure. Some patients would be better served with a non-surgical radio frequency (RF) microneedling or CO2 laser. Other patients may require a traditional neck lift to reach their goals.
Medical evaluation: The second part of the consultation is to determine if the patient is medically suitable to undergo the procedure. As the procedure is performed under local anesthesia, most patients will qualify. Contraindications may include being on a blood thinner or an aversion to procedures without general anesthesia. If someone prefers general anesthesia, chin and neck liposuction can be scheduled at the surgery center.

Scheduling the procedure: Once the patient has decided to proceed, a procedure date will be selected. Fridays are popular days to undergo chin and neck liposuction in the office. This allows patients to recover over the weekend and most can go to work on Monday.
The day of the procedure: Dr. Domanski will once again confirm your goals, and a consent form will be signed. Some patients may choose to take medication to reduce anxiety. If medication is needed, we can prescribe this. However, we have found that most patients do not need this.
During the procedure: The procedure starts by administering a numbing injection under the chin and below the ears. Then a fluid called tumescent is injected into the area to be treated. Tumescent fluid is isotonic – which means it is designed to have the same salt concentration as fluid in our body. Tumescent fluid also contains medication (lidocaine) to reduce pain and medication to reduce bruising.
Tumescent fluid works by reducing pain and bruising, but also helping separate the fat from the surrounding tissue in the neck. Once the tumescent fluid has had enough time to instill, usually 10 minutes, the fat reduction procedure begins.
A cannula is inserted through one of three access points to remove the fat with gentle suction. Some of the tumescent fluid is also removed in this way.
At the completion of the procedure, the access points are closed with absorbable sutures. Usually, only 3 sutures in total are required. A compression bandage is applied, and patients go home.

Patients may shower the day after surgery. Liposuction generally only requires only 3-4 days of down time. You will have to wear a compression garment for the first 4 weeks following the procedure. Full results will be seen in approximately three months. The neck skin will be tighter after neck liposuction. This occurs over weeks and months as swelling resolves, and the collagen is deposited and remolded in the neck. This remodeling of collagen results in skin contraction and tighten neck skin.

Scarring – Incisions are typically small and usually well-hidden. However, individual healing characteristics can affect the extent of scarring. We provide our patients with a scar cream to minimize the risk of poor scarring.
Skin Laxity – In patients with poor skin elasticity, loose skin may persist after fat removal. Additional skin-tightening treatments may be recommended.

The best treatment for neck fat depends on how much fat is present and your skin’s elasticity. Chin and neck liposuction is one of the most effective ways to permanently remove excess fat if you have good skin quality. In some cases, non-surgical options like RF microneedling can be used as alternatives or complements. Another non-surgical treatment is Kybella injection. However, Kybella has significant cost, requires repeat treatments, and has a fair amount of swelling during recovery. Unlike med-spas run by non-surgeons, we are a plastic surgery office. Therefore, we can offer neck liposuction. Infact, we prefer neck liposuction over Kybella due to its predictability.
Non-surgical options are easier to administer. For some patients, radiofrequency microneedling may be the ideal choice due to its focus on skin tightening. CO2 laser can help improve skin texture by reducing fine lines. Neck liposuction is considered surgical treatment and requires more skills on behalf of the practitioner. Open surgical neck de-fating is performed under general anesthesia as part of or separate from a neck lift. Thus, open surgical neck de-fating requires mores skill, more downtime, but the results can also be more dramatic.
The best procedure is different for each patient. A young patient with extra fat may opt for neck liposuction in the office due to its low downtime and predictable results. An older patient may choose to undergo microneedling if skin contracture is their primary concern. Patients with fine lines may be best treated with CO2 laser. A patient with a thick neck may be best served by operative fat removal under anesthesia.
Better depends on the patient. This is like asking if an omelet is better than a steak. The omlet is better for breakfast, and the steak is probably better for dinner.
Excess skin can be removed like a tailor removes excess fabric. When excess skin is removed, the remaining skin is tighter. This requires a traditional neck lift. Chin liposuction can be combined with a neck lift or done separately. Chin liposuction can be done under local anesthesia in the office- saving the cost of a surgery center and anesthesiologist. Chin liposuction is better if the patient has only moderately loose skin that will tighten ince the fat is removed. The right approach depends on your skin quality, chin fat, and aesthetic goals.
You can expect some swelling, bruising, and tightness in the first few days. Your neck may feel tight, but it will look natural. Most patients return to work the next day or within a few days. A compression garment is usually worn to support healing and contouring. Final results develop over several weeks as swelling subsides.
Patients go home the same day. The next day, patients can shower and resume normal light activities. This may include shopping, watching movies, and going for a walk. Bruising decreases over the next couple of days. Some patients do not experience bruising. Swelling can take several weeks to fully resolve. Full results are typically visible within 1–3 months, depending on your body’s healing response.
